PTO Fund Request Form

TEACHERS: Use this form to request funding from the PTO.

  1. Please fill out form completely.
  2. Place a copy in PTO mailbox AND in Ms. Rodriguez' mailbox ( 2 copies).
  3. Once approved Ms. R will contact you OR PTO board member will.

PTO Check Request Form

TEACHERS: Use this form to request reimbursement from the PTO for items already paid for.

  1. Please fill out form completely.
  2. Place a copy in PTO mailbox AND in Ms. Rodriguez' mailbox ( 2 copies).
  3. Once approved you will receive a check.
